TCC - Admission Criteria

In order to be admitted to the Transitional Care Center, the patient must meet all of the following requirements:

• need daily skilled nursing care and/or specialized therapy
• have had a prior acute hospitalization of 72 hours or more (unless waived by payor source)
• be medically stable
• require inpatient services, rather than home health or outpatient services
• be willing to follow the discharge plan developed jointly by the interdisciplinary team and the patient
