Senior Assessment Program

The Senior Assessment Program at The Reading Hospital Medical Group Center for Aging is designed to help older individuals and their families identify these challenges. Based on the results of a thorough screening, options can be evaluated — from medication management to long-term care — to assure a high quality of life throughout the senior years.
