Specialized Care for Wounds and Ostomies

It’s not uncommon for home care patients to have wounds that don’t heal easily. These could be pressure ulcers in bed-bound individuals, ulcers on the feet of diabetic patients, and slow-to-heal surgical incisions. Fortunately, The Reading Hospital Home Care team includes a registered nurse with specialized training in wound care, as well as in ostomy and incontinence care.

Telemedicine

We are pleased to offer telemedicine, a home monitoring device that helps you stay on top of your own care. With this technology, your vital signs, weight, and pulse oximetry are transmitted to the Home Care office, where a specialist reviews the information and relays it to your physician, who provides follow up if needed. In addition to improving patient compliance and satisfaction, telemedicine reduces re-hospitalization and emergency visits.

Home Safety Assessment for Joint Care Patients

We work in close conjunction with The Reading Hospital Joint Care Center to assure continuity of care and to foster positive outcomes from joint replacement surgery.

Our therapists provide home safety assessments prior to admission to the Join Care Center to make recommendations that lead to faster recovery and increased independence. These skilled professionals identify safety issues, equipment needs, and mobility concerns prior to surgery so everything is in place when the patient is ready to return home.
