Pathway to Surgery

The first steps…

1. Attend a free informational seminar. 
2. Complete the patient registration and seminar survey forms, and turn them in at the end of the seminar.
3. Have your insurance benefits verified by our Patient Representative, who will contact you generally within five business days of attending the seminar. When she contacts you, she will give you an appointment with your surgeon, a link to complete an online medical record, and online videos to view.

Meeting your surgeon…

1. Be sure that you have viewed the online videos given to you by the Patient Representative, and bring a list of any questions that you may have.
2. Complete all paperwork (online and/or paper copies) prior to your appointment.
3. Bring your insurance card, co-pay, photo ID and referral (if your insurance company requires one).

Next steps…

1. Complete all required consults and testing (we will help you to arrange this).
2. Meet with your surgeon for final visit, during which time you will receive your surgery date.
3. Submit all documentation to your insurance company for approval for surgery (we will do this for you).

Once you are approved…

1. Complete preadmission testing.
2. Review website information regarding your diet/supplement guidelines.
